Been done for speeding. Been sent a letter offering three options:

1. Attend a course costing 65 - 125

2. Pay a fixed penalty of 60 and 3 points

3. Attend court.

I want Option 2. but there's no way of asking for it in the letter apart
from waiting 14 days for Option 1. to expire.

Do I just wait for 14 days?

Go on the course. It sounds like you need it.

Get someone to read the form for you if you can't understand it.
Everything is clear enough, so that might explain why you couldn't
match what was on your speedometer to the road signs showing the
speed limit. I'm not sure a course would be of much use to a driver
like you, maybe you need a further driving test and eyesight test.

Is there a potential difference in the effect on your insurance with 1 and
2?

(I know that most insurance companies will disregard the first speeding
conviction, so what will be the effect of a second one with either option?)

Yep.
You don't have to declare 1)
